The permit holder shall require food employees to report to the Person In Charge information about their health and activities as they relate to diseases that are transmissible through food. A food employee shall report the information in a manner that allows the Person In Charge to reduce the risk of foodborne disease transmission. AMC 16.60.130(2-201.11.A) The Person In Charge shall notify the regulatory authority when a food employee is: (1) Jaundiced; or (2) Diagnosed with norovirus, Hepatitis A, Shigella, E. coli, or Salmonella Typhi. AMC 16.60.130(2-201.11.B) ***OWNER/PERMIT HOLDER WAS NOT ABLE TO PROVIDE VERIFIABLE INFORMATION INDICATING FOOD WORKERS HAVE BEEN PROPERLY NOTIFIED OF REPORTING REQUIREMENTS AS NOTED ABOVE. AN EMPLOYEE HEALTH AGREEMENT WAS LEFT WITH THE OWNER/PERMIT HOLDER. ENSURE EMPLOYEE HEALTH AGREEMENT IS MAINTAINED ON SITE AND AVAILABLE TO INSPECTORS WHEN REQUESTED.
A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees and shall be clearly visible to food employees. AMC 16.60.170(6-301.14) ***NO HANDWASHING SIGN OR POSTER POSTED AT HANDWASHING SINK. ENSURE TO FOLLOW DIRECTIVE ABOVE.
A plumbing system shall be repaired according to law; and maintained in good repair. AMC 16.60.160(5-205.15) ***OBSERVED IN THE DOWNSTAIRS KITCHEN; BACK FLOW OCCURRING UNDER 3 COMPARTMENT SINK WHEN WAREWASHING MACHINE DRAINS. THE OWNER/PERMIT HOLDER STATED THAT THE 3 COMPARTMENT SINK AND WAREWASHING MACHINE DRAINS INTO THE SAME DISCHARGE PIPE. PER THE OWNER/PERMIT HOLDER A PLUMBER IS COMING TODAY TO ASSESS THE 3 COMPARTMENT SINK. MAKE NECESSARY REPAIRS TO ENSURE THAT 3 COMPARTMENT SINK AND THE DISHWASHER PROPERLY DRAIN. At least 1 service sink or 1 curbed cleaning facility equipped with a floor drain shall be provided and conveniently located for the cleaning of mops or similar wet floor cleaning tools and for the disposal of mop water and similar liquid waste. AMC 16.60.160(5-203.13) ***FACILITY DOES NOT HAVE A MOP SINK. PROVIDE A COMPLIANCE SCHEDULE FOR INSTALLING THE MOP SINK. INSTALLATION NEEDS TO BE COMPLETED BY THE END OF THE YEAR 12/31/2025.
