ESTABLISHMENT HAS NOT YET OPERATED, PERMIT HOLDER MUST REPLACE THE CHEMICAL SANITIZER WITH ONE THAT IS INTENDED FOR USE ON FOOD CONTACT SURFACES. FOLLOW DIRECTIVE ABOVE.

AMC 16.60.170(6-201.11) **OBSERVED MULTIPLE PIPES COMING THROUGH THE WALLS AND/OR THE FLOORS THAT WERE NOT SEALED AROUND THE PIPES. SEAL THE AREA AROUND THE AFOREMENTIONED PIPING. ENSURE ALL PIPING THAT GOES THROUGH ANY WALLS, FLOORS OR CEILINGS ARE SEALED AS DIRECTED ABOVE. **OBSERVED CHIPS, CRACKS, HOLES ON THE WALLS AND FLOORS THROUGHOUT THE ESTABLISHMENT. PERMIT HOLDER MUST REPAIR ALL CHIPS, CRACKS, AND HOLES SO THAT THE WALL AND FLOOR SURFACES ARE SMOOTH AND EASILY CLEANABLE AS DIRECTED ABOVE.
