    A handwashing sink shall be maintained so that it is accessible at all times for employee use. AMC 16.60.160(5-205.11.A) ***Facilities must be maintained in a condition that promotes handwashing and restricted for that use. Convenient accessibility of a handwashing facility encourages timely handwashing which provides a break in the chain of contamination from the hands of food employees to food or food-contact surfaces. Sinks used for food preparation and ware washing can become sources of contamination if used as handwashing facilities by employees returning from the toilet or from duties which have contaminated their hands. ***OBSERVED HANDWASH SINK AT THE COFFEE BAR FILLED WITH DIRTY DISHES. PERSON IN CHARGE WAS INSTRUCTED THAT HANDWASH SINKS CANNOT BE USED FOR ANYTHING EXCEPT HANDWASHING AND MOVED THE OBSTRUCTION. ENSURE HANDWASH SINKS REMAIN ACCESSIBLE AT ALL TIMES TO PROMOTE PROPER HANDWASHING.

AMC 16.60.140(3-603.11.A) Disclosure shall include: (1) A description of the animal-derived foods, such as “oysters on the half shell (raw oysters),” “raw-egg Caesar salad,” and “hamburgers (can be cooked to order);” or (2) Identification of the animal-derived foods by asterisking them to a footnote that states that the items are served raw or undercooked, or contain (or may contain) raw or undercooked ingredients. AMC 16.60.140(3-603.11.B) Reminder shall include asterisking the animal-derived foods requiring disclosure to a footnote that states: (1) Regarding the safety of these items, written information is available upon request; (2) Consuming raw or undercooked meats, poultry, seafood, shellfish, or eggs may increase your risk of foodborne illness; or (3) Consuming raw or undercooked meats, poultry, seafood, shellfish, or eggs may increase your risk of foodborne illness, especially if you have certain medical conditions.     Outer openings of a food establishment shall be protected against the entry of insects and rodents. AMC 16.60.170(6-202.15) **OBSERVED LIGHT ENTERING THE ESTABLISHMENT THROUGH THE EDGES AROUND THE DOOR AT THE REAR EXIT TO THE ESTABLISHMENT AND AROUND THE EDGES OF THE MAIN FRONT DOOR TO THE ESTABLISHMENT. WHEN LIGHT IS VISIBLE THROUGH THE EDGES AROUND THE DOOR A DOOR SWEEP OR OTHER MEANS OF PROTECTION MUST BE INSTALLED TO PROTECT AGAINST THE ENTRY OF INSECTS AND RODENTS. PERMIT HOLDER MUST REPAIR THE EDGES AROUND THE DOORS TO ENSURE THE DIRECTIVE ABOVE IS FOLLOWED AT ALL TIMES.

AMC 16.60.160(5-205.15) **OBSERVED MULTIPLE FAUCETS THROUGH THE ESTABLISHMENT THAT HAVE MISSING OR INCOMPLETE AERATORS, THE INCOMPLETE AERATORS DID NOT HAVE THE FAUCET SCREENS. PERMIT HOLDER MUST INSTALL OR REPAIR THE AERATORS THROUGHOUT THE ESTABLISHMENT. ENSURE ALL FAUCETS HAVE AERATORS AND ARE INSTALLED AND MAINTAINED AS DIRECTED ABOVE.

PERMIT HOLDER MUST PROVIDE A COVERED RECEPTACLE IN A RESTROOM INTENDED FOR USE BY FEMALES. FOLLOW THE DIRECTIVE ABOVE.

    Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. AMC 16.60.170(6-201.11) **OBSERVED MULTIPLE HOLES IN THE WALLS IN THE MAIN KITCHEN AND MULTIPLE CLEAN IN PLACE METAL SHELVES ON THE WALLS THAT WERE NOT SEALED BETWEEN THE WALL AND THE SHELVES. PERMIT HOLDER MUST REPAIR ALL CRACKS, CHIPS, OR HOLES IN THE WALLS AND MUST ENSURE ANY SHELVES THAT ARE SECURED TO THE WALLS ARE SEALED BETWEEN THE WALL AND THE SHELVE SO THAT NO CRACKS OR CREVICES ARE PRESENT. ENSURE ALL WALLS AND SHELVES ARE CONSTRUCTED AND MAINTAINED AS DIRECTED ABOVE.
