The permit holder shall require food employees and conditional employees to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food. A food employee or conditional employee shall report the information in a manner that allows the person in charge to reduce the risk of foodborne disease transmission, including providing necessary additional information, such as the date of onset of symptoms and an illness, or of a diagnosis without symptoms, if the food employee or conditional employee: 1. Has any of the following symptoms: (a) Vomiting, (b) diarrhea, (c) jaundice, (d) sore throat with fever, (e) a lesion containing pus such as a boil or infected wound that is open or draining and is (i) On the hands or wrists, unless an impermeable cover such as a finger cot or stall protects the lesion, and a single-use glove is worn over the impermeable cover. (ii) On exposed portions of the arms, unless the lesion is protected by an impermeable cover or (iii)On other parts of the body, unless the lesion is covered by a dry, durable, tight-fitting bandage. 2 .Has an illness diagnosed by a HEALTH PRACTITIONER due to: (a) Norovirus, (b) Hepatitis A virus, (c) Shigella spp., (d) PSHIGA TOXIN-PRODUCING ESCHERICHIA COLI, (e) Salmonella Typhi; or (f) nontyphoidal Salmonella; AMC 16.60.130(2-201.11.A) THE PERSON IN CHARGE MUST ENSURE THAT ALL FOOD EMPLOYEES UNDERSTAND THEIR OBLIGATION TO REPORT HEALTH-RELATED INFORMATION TIED TO DISEASES TRANSMISSIBLE THROUGH FOOD. THIS REPORTING MUST BE VERIFIABLE. A SIMPLE WAY TO MEET THIS REQUIREMENT IS FOR ALL FOOD EMPLOYEES TO COMPLETE A FOOD EMPLOYEE REPORTING AGREEMENT AND ENSURE THE SIGNED DOCUMENT IS AVAILABLE WHEN REQUESTED. A FOOD EMPLOYEE REPORTING AGREEMENT IS AVAILABLE FOR DOWNLOAD AT WWW.MUNI.ORG/FOODINFO. ENSURE EMPLOYEE HEALTH AGREEMENT IS MAINTAINED ON SITE AND AVAILABLE TO INSPECTORS UPON REQUESTED.

AMC 16.60.170(6-202.15) **OBSERVED LIGHT ENTERING THE ESTABLISHMENT THROUGH THE DOOR SWEEP AT THE BOTTOM OF THE BACK DOOR OF THE ESTABLISHMENT. PERMIT HOLDER MUST REPAIR OR REPLACE THE DOOR SWEEP SO THAT NO LIGHT IS ABLE TO ENTER THE ESTABLISHMENT THROUGH THE EDGES OF THE DOOR. IF LIGHT IS ABLE TO ENTER THE ESTABLISHMENT THROUGH THE EDGES AROUND AN OUTER OPENING, THE OPENING IS NO LONGER PROTECTED. ENSURE DIRECTIVE ABOVE IS FOLLOWED AT ALL TIMES.

PERMIT HOLDER MUST REPAIR OR REPLACE THE AFOREMENTIONED SEAL IF CONTINUING TO USE THE REFRIGERATOR WITH THE GLASS DOOR. FOLLOW DIRECTIVE ABOVE.

AMC 16.60.160(5-103.11.A) **THE HOT WATER HEATER CURRENTLY INSTALLED DOES NOT HAVE THE SUFFICIENT CAPACITY TO MEET THE PEAK WATER DEMANDS OF THE FOOD ESTABLISHMENT. THIS WAS DETERMINED BY THIS INSPECTOR RUNNING THE HOT WATER CALCULATIONS FOR THE ESTABLISHMENT WHILE ON-SITE DURING THIS INSPECTION. THE PERMIT HOLDER WAS SHOWN THE HOT WATER CALCULATIONS THAT WERE COMPLETED AND IT WAS DISCUSSED THAT THE HOT WATER SYSTEM WILL NEED TO BE UPGRADED OR REPLACED TO MEET THE PEAK WATER DEMANDS FOR THE ESTABLISHMENT. THE PERMIT HOLDER STATED THEY WILL BE WORKING ON REPLACING THE CURRENT HOT WATER HEATER OR ADDING A HOT WATER HEATER TO THE CURRENT SYSTEM. PERMIT HOLDER MUST WORK WITH THIS INSPECTOR AS THEY ARE REPLACING THE HOT WATER HEATER OR ADDING ADDITIONAL HOT WATER HEATERS, COMMUNICATING AS THE SYSTEM IS BEING WORKED ON AND WHEN THE WORK HAS BEEN COMPLETED. IF MORE TIME IS NEEDED TO COME INTO COMPLIANCE WITH THIS REQUIREMENT THE PERMIT HOLDER MUST COMMUNICATE THE UPDATED TIMELINE TO THIS INSPECTOR OR THE DISTRICT SANITARIAN. FOLLOW DIRECTIVE ABOVE.

PERMIT HOLDER MUST INSTALL AN AERATOR ON THE MOP SINK. PERMIT HOLDER MUST ALSO ENSURE ALL FAUCETS HAVE AERATORS AND ARE INSTALLED AND MAINTAINED AS DIRECTED ABOVE. **OBSERVED WATER LEAKING FROM THE PIPES CONNECTED TO THE FOOD PREPARATION SINK. PERMIT HOLDER STATED PERMIT HOLDER STATED THEY WILL CALL THEIR PLUMBER TO REPAIR THE LEAKING SINK. FOLLOW DIRECTIVE ABOVE.
